A practical standard for logging family contact so the record stays short and factual, and is actually useful at the next meeting. The aim is a log a teammate, administrator, or the family could read without surprise, where you can scan a few months and see exactly what was shared, when, and through which channel.

Capture the essentials, not a transcript

For each contact, record who reached whom, the channel, the topic, and any next step. That is usually enough. Skip long subjective narratives unless the team genuinely needs that context, because over-documenting makes the log harder to scan and harder to keep up.

Write in neutral, respectful language

Write every entry as if the family, an administrator, or an advocate may read it later, because any of them might. Stick to observable facts and the agreed next step rather than your read on the family's tone or intent. Neutral wording lets the same log support collaboration in a calm meeting and hold up in a tense one.

Separate family-facing notes from internal interpretation

Keep what you said to the family distinct from your private interpretation of the situation. Mixing the two is how a sensitive judgment ends up somewhere it should not be, especially once exports and sharing enter the picture. When in doubt about whether a note belongs in a family-facing view, leave it out and preview before sharing.

Review the log before meetings

Before an IEP meeting or support review, scan the communication log for patterns: missed contacts, a preferred language, slow responses, or a concern the family has raised more than once. Those patterns often explain a data trend or set up the conversation better than the numbers alone. Reviewing first also means you are not surprised by your own record mid-meeting.
