A checklist for documenting behavior supports across MTSS tiers so a decision to move a student up or down a tier is backed by data, not impressions. It works through naming the current tier and support, collecting the data that justifies it, reviewing at a checkpoint, and recording the decision with its evidence.
Name the current tier and the specific support
Write down which tier the student is in and the exact support in place, such as a check-in routine, a small-group intervention, or an individualized plan. Vague entries like receiving Tier 2 are not enough; name the intervention precisely. A specific record is what lets the next team member continue the support without guessing.
Collect the data that justifies the tier
Decide what data shows whether the current support is working: a daily chart goal, frequency of incidents, or ABC notes for a behavior the team is trying to understand. Log it consistently for a defined window so the tier decision rests on a real trend. Thin or inconsistent data is the most common reason a tier-change request gets pushed back.
Set the review checkpoint before you start
Agree up front on when you will review and what a move up or down a tier would require. Setting the checkpoint and the criteria before collection keeps the eventual decision honest instead of retrofitted. Tie the review to the same trend you are logging so the conversation has evidence in front of it.
Record the decision and its evidence together
When the team decides to intensify, maintain, or fade a support, record the decision next to the data that drove it. A tier change that holds up shows the support tried, the data collected, and the reasoning, not just an outcome. Keeping the chart trend and any incidents close to the decision means you can reconstruct the why later without rebuilding it from memory.
Carry the documentation toward IEP or evaluation if needed
If a student is not responding across tiers, the same documented trail supports a referral or evaluation conversation. Because the daily data and incidents already live next to the student, you are not starting evidence collection from scratch. District and state MTSS and eligibility procedures remain the source of truth for what counts and when.
