Frequency Data Sheet
A free printable frequency count sheet for IEP and behavior goals measured as how many times something happens in a set period. Free to print, copy, and hand to a paraeducator. No account, no email address.
Use this sheet when the goal reads like
- “will initiate a peer interaction 4 or more times per class period”
- “will have fewer than 3 instances of calling out per 30-minute lesson”
Frequency only means something against a known window. Keep the window the same length where you can, and when you cannot, record the actual times so the comparison can be made per minute later.

- Write the start and end time before you start counting. A count without a window is not a rate and cannot be compared to another day.
- Tally as it happens. Reconstructing a count at the end of the period consistently undercounts.
- If the period gets cut short, record the real end time rather than dropping the session.
