Trial-Based IEP Data Sheet
A free printable trial-by-trial data sheet for IEP goals written as a number of correct trials out of a total, with a column for prompt level. Free to print, copy, and hand to a paraeducator. No account, no email address.
Use this sheet when the goal reads like
- “will identify the main idea in 8 out of 10 trials”
- “will solve two-digit addition problems with 80% accuracy across 3 consecutive sessions”
Trial data is the most common way IEP goals are written and the easiest to distort. Two sessions of 4 trials each are not the same as one session of 8, so keep them on separate lines even when the total is the same.

- Record the trials you actually ran, not the trials you planned. A session of 6 is data; a session you rounded up to 10 is not.
- Write the prompt level for the session as a whole, or the highest level of support you gave if it varied.
- Percentage is correct divided by total. Leave the math for later rather than doing it at the table.
